Group Homes

Program Summary

The Group Homes Program provides a shared residence for individuals with mental illness that are working towards living independently in the community. No staff on-site; support is provided through intensive case management services and interaction on a regular basis. 

Collateral case management services provides:

  • Assertive outreach,
  • Assessment,
  • Information and education,
  • Support, coordinate services and links to resources that an individual may need,
  • Service plan based on individual needs,
  • Supportive counseling,
  • Crisis-planning, crisis intervention and crisis prevention,
  • Assistance with symptom and behavior management,
  • Medication support,
  • Skill-development,
  • Family support and education,
  • Advocacy and
  • Public/professional education.

Service Hours

The regular scheduled support hours for the services are between 8am-8pm Mon-Th. 8-5pm on Fridays. Arrangements will be made to meet individual needs as requested.
