Intensive Case Management
Program Summary
ICM promotes independence and improved quality of life with the primary focus on people with a serious mental illness through the coordination of appropriate services and the provision of ongoing support as needed using a recovery model focus. The direct involvement of the individual and the development of a supportive relationship between the community support worker and the individual are integral to developing personal recovery goals. Case management is a collaborative service consisting of interrelated processes to support individuals in their efforts to achieve optimal health and independence in a complex health, social, and physical environment. The overall goal of case management is to improve the individual’s ability to function in the community. A broad range of direct interventions are provided and tailored to the individual’s needs, including but not limited to:
- Assertive outreach,
- Assessment,
- Information and education,
- Support, coordinate services and links to resources that an individual may need,
- Service plan based on individual needs,
- Supportive counseling,
- Crisis-planning, crisis intervention and crisis prevention,
- Assistance with symptom and behavior management,
- Medication support,
- Skill-development,
- Family support and education,
- Advocacy and
- Public/professional education.
